Please check your email inbox for a copy of your submission.

Of course, we understand a primary concern at this stage is how you’re going to get paid! To get that important stuff ironed out quickly, you’ll need to email the following documents (as applicable) to accounting@bothwelltransport.com:

Load Docs:

  • Invoice with company name, address, payment terms and Bothwell Transport load number
  • Rate confirmation
  • Legible bill of lading/proof of delivery (if originals are needed please mail them to: Bothwell Transport, PO Box 12499, Glendale, AZ 85318)

Additional Documents:

  • HAZMAT Certification
  • Diverse-Owned Business Certification

Also, if you were unable to upload the following documents (as applicable), please email the following Carrier Docs to dispatch@bothwelltransport.com:

Carrier Docs

  • Operating Authority Letter
  • Company W-9
  • Certificate of Insurance
  • Voided Check (if requesting direct deposit)
  • Factoring Notice of Assignment
